Job summary

The Riverside Premier Rehabilitation & Healing Center is seeking an Experienced Physical Therapist to assess and treat patients in a rehabilitation setting. The PT will develop and administer treatment plans, document patient progress, and educate patients and families to optimize outcomes.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid professional license and state registration.
• Experience supervising Physical Therapy Assistants.
• Geriatric care experience preferred.
• Proficient in patient documentation.
• Ability to document evaluations, progress notes, recertifications and discharge summaries.

Responsibilities

• Assess referred patients and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity and tolerance.
• Document patient condition and progress; educate patients and families in therapy.
• Collaborate with all disciplines to provide patient-centered care.
• Maintain confidentiality and adhere to HIPAA and residents' rights.
