Therapy Management Corporation (TMC) in Arizona seeks a Licensed Physical Therapist to empower adult/geriatric patients through compassionate, patient-centered therapy focused on independence and quality of life. You’ll provide services in a collaborative clinical setting with clinical autonomy and support.

Responsibilities include evaluating patients, developing individualized treatment plans, documenting progress, and coordinating with nurses and caregivers to ensure continuity of care.

Qualifications

• Licensed Physical Therapist (PT) or eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy to support recovery and enhance quality of life in older adults.
• Help patients restore functional mobility and safely perform daily activities.
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to drive outcomes and continuity of care.
• Track progress, document clinically, and adapt plans based on needs.
