Job summary

Qualifications

• Licensed Physical Therapist (PT) in-state license
• Graduate of an accredited Physical Therapist Program
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance and functional independence
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
• Deliver hands‑on therapy interventions to support recovery and independence in older adults
• Help patients restore functional mobility and safely perform daily activities with greater confidence
• Partner with interdisciplinary teams, caregivers and nursing staff to drive meaningful patient outcomes
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans as needed
