Job summary

Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist (PT) to empower adult and geriatric patients through compassionate, patient-centered care. In this role, you will work to improve independence, mobility, safety and overall quality of life, while maintaining clinical autonomy in a collaborative clinical environment.

Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long‑term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults.
• Help patients restore functional mobility and safely perform daily activities with greater confidence and independence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals.
