Qualifications

• Licensed PT or license eligible to practice in the state.
• Graduate of an accredited Physical Therapist program.
• Interest or experience in skilled nursing, long-term care or geriatric rehab.
• Passion for patient-centered care and improving life quality for older adults.

Responsibilities

• Evaluate patients and provide personalized PT treatments to improve mobility, strength, balance, and independence.
• Develop and implement patient-centered treatment plans for long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery and endurance in older adults.
• Help patients restore functional mobility for daily activities with confidence and independence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful outcomes and continuity of care.
• Track progress, document clinically, and adapt plans based on rehabilitation goals.
