Therapy Management Corporation (TMC) is seeking a licensed Physical Therapist to provide patient-centered therapy to adults and older adults in a geriatric rehabilitation setting. You will collaborate with interdisciplinary teams to improve mobility, independence and quality of life.

Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility and independence.
• Develop and implement patient-centered treatment plans for long-term rehabilitation goals.
• Deliver hands-on therapy to support recovery and enhance quality of life in older adults.
• Help patients restore functional mobility and perform daily activities with confidence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive outcomes.
• Track progress and maintain accurate clinical documentation; adapt plans as needed.
