Job summary

Trinity Health PACE is seeking a Primary Care Physician to provide comprehensive medical care for PACE participants at Kinder Park. The PCP will lead assessments, develop and implement care plans, and work with an interdisciplinary team to optimize outcomes.

Responsibilities include coordinating specialist referrals, managing nursing home patient care, and ensuring proper documentation and quality assurance across the care continuum in a patient-centered setting.

Qualifications

• MD or DO with active license to practice in the relevant state.
• Board certification in Internal Medicine or Family Practice is required; geriatrics certification preferred.
• Minimum 1 year experience with frail or elderly population.

Responsibilities

• Perform initial and ongoing patient assessments and develop and implement care plans.
• Collaborate with the Interdisciplinary Team and coordinate care across settings.
• Refer patients to medical specialists and manage nursing home care in collaboration with staff.
• Monitor outcomes and adjust care plans; complete documentation and coding.
• Contribute to policy development, training, and quality assurance.
