Trinity Health Mercy LIFE PACE program in Philadelphia seeks a Primary Care physician to deliver comprehensive medical care to frail elders as part of an interdisciplinary team. The physician will assess, plan, and coordinate care for participants, coordinating with specialists and hospital care when needed.

Requirements include MD/DO, active license, CDS and DEA, BLS, and board certification in Internal Medicine with geriatrics preferred. Travel within the region may be required.

Qualifications

• Doctorate Degree (MD or DO) and license to practice in state required.

Responsibilities

• Provide primary medical care to PACE participants in coordination with the interdisciplinary team.
