Job summary

TRU Community Care is seeking a Nurse Practitioner or Physician Assistant to join the PACE Primary Care Clinic in Lafayette, CO.

You will deliver comprehensive primary care to geriatric participants enrolled in PACE and may care for hospice patients as part of a collaborative care model.

Qualifications

• 2+ years of primary care experience.
• Unencumbered Colorado NP/PA license.
• Current CDS registration and DEA license.
• Eligible for Medicare/Medicaid provider credentials.
• $1M/$3M professional liability insurance; TRU reimburses premiums.
• Valid driver’s license with reliable transportation.

Responsibilities

• Provides diagnosis, treatment, and ongoing management for PACE participants and hospice patients.
• Provides primary care in clinic and, when needed, in the participant's home.
• Participates in after-hours call rotation.

Title: Nurse Practitioner / Physician Assistant – PACE Primary Care
Location:Lafayette, CO
Schedule:Monday-Friday, 8am-5pm
On Call: Sun–Wed or Thurs–Sat, depending on rotation. Phone-only; no travel required.
Pay:$120,087 to $144,988 per year

JOB SUMMARY

The Provider works in collaboration with other providers and interdisciplinary teams to deliver primary care and other health services to geriatric participants enrolled in PACE. The Provider may also participate in the clinical call rotation for TRU Community Care and provide care to hospice patients.

This position offers the opportunity to build meaningful, long-term relationships with older adults while providing coordinated, person-centered care in both clinic and community settings.

ESSENTIAL DUTIES

• Provides diagnosis, treatment, and ongoing management services for PACE participants and hospice patients as directed.
• Provides primary care in the clinic and, when needed, in the participant's home.
• Responds to urgent clinical needs during regular work hours and participates in the shared after-hours call rotation.
• Participates actively in interdisciplinary team conferences, helping to develop and revise participant/patient care plans and serving as a medical resource in determining approaches to individual participant/patient care.
• Collaborates with physicians, nurses, social workers, therapists, and other members of the interdisciplinary team to provide coordinated care.
• Provides palliative care and symptom management as appropriate.
• Works in a safety-conscious manner and follows safe work practices to protect participants, patients, coworkers, and self.

EDUCATION AND QUALIFICATIONS

• Nurse Practitioner (NP) or Physician Assistant (PA) with2+ years of primary care experience.
• Unencumbered medical license in Colorado (NP or PA).
• Current controlled substance registration and DEA license number.
• Eligible for or currently possess the required Medicare/Medicaid provider credentials.
• Eligible to bill insurance for palliative care and symptom management visits.
• Experience working with frail and/or elderly patients preferred.
• Must have $1M/$3M professional liability insurance; TRU will reimburse the premium costs.
• Must possess a valid driver's license, provide proof of insurance meeting Colorado state minimums, and have reliable transportation for required daytime participant visits.
• Must comply with agency health screening requirements, including medical clearance for communicable diseases prior to direct participant contact and proof that all required immunizations are up to date prior to direct participant contact.
