Overview

Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on call or weekends)!

We are seeking a full‑time (40 hours per week) provider for a split position between our Majestic Care of Hopemont Center located in Terra Alta, WV and Cortland Acres, located in Thomas, WV. This position can be filled by a nurse practitioner or physician assistant.

The salary range for this position is $109,000 – $124,000 per year.

Responsibilities

• The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
• The Advanced Practice Provider collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities.
• The Advanced Practice Provider collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care.
• The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
• The Advanced Practice Provider works collaboratively with Center Leadership to achieve AlignMed Partners’ mission of improving the lives we touch through the delivery of high‑quality health care and everyday compassion.

Qualifications

• Graduate from an accredited School of Nursing program, licensed as a registered nurse.
• Certified as a Nurse Practitioner by completing an accredited Nurse Practitioner Program with a Master’s Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks may be considered based on experience and business need.
• Physician Assistants must graduate from a program accredited by the ARC‑PA, pass the PA National Certifying Exam (PANCE) to use the PAC designation, have PA state licensure, and hold NCCPA certification.
• Minimum of two years experience working with adult or geriatric population is preferred.
• In states where practice allows, APRNs must have an unrestricted professional license and DEA for each state in which they practice; PAs likewise.

Restrictions apply based on collective bargaining agreements, applicable state law, and factors such as pay classification, job grade, location, and length of service. Posted Salary Range: USD $109,000.00 – $124,000.00 per year.
