Valir Health is seeking an Advanced Practice Registered Nurse or Physician Assistant to deliver medical care to PACE participants in Oklahoma City. The provider works with physicians, therapy, social services, and transportation to monitor care plans in compliance with CMS and state regulations.

The role requires an OK APRN/PA license, DEA/OBNDDC credentials, and CPR. Prior experience with frail elderly populations is preferred.

Qualifications

• Current APRN or PA license in Oklahoma.
• Licensed in Oklahoma with no restrictions.
• DEA registration and OBND DC licensure.
• Experience with frail and elderly population or approved training.
• Current CPR certification.

Responsabilités

• Performs comprehensive exams on enrollment and every six months per CMS regulations.
• Diagnose and treat acute illness in participants.
• Provide follow-up care to manage chronic disease.
• Engage the participants' support system in medical care.
• Rounds and attends to institutionalized participants.
• Coordinate care with specialty providers and follow-up.
• Assist IDT with care plan development and revisions.
• Serve on committees as required by Medical/Program Directors.
• Maintain accurate and timely documentation.
• Other duties as assigned.
