Job summary
Valir PACE Foundation is seeking an Advanced Practice Provider (APRN/PA) to deliver medical care within an interdisciplinary team for our PACE participants. You will determine and monitor care plans in line with CMS regulations and participate in comprehensive enrollment exams and ongoing follow-ups.
The role requires current Oklahoma APRN or PA licensure, CPR certification, and experience with frail elderly populations.
Qualifications
- Current APRN or PA license.
- Certified to practice in the State of Oklahoma.
- Licensed by OBNDDC and the DEA.
- CPR certification.
- Experience with frail and elderly population is preferred.
Responsibilities
- Performs comprehensive exams at enrollment and every six months.
- Diagnose and treat acute illness.
- Provide follow-up care for chronic disease.
- Engage the participant's support system in medical care.
- Coordinate care with specialty providers and document accurately.
