VALIR PACE LLC in Oklahoma seeks an Advanced Practice Registered Nurse or Physician Assistant to join an Interdisciplinary Team delivering comprehensive medical care to PACE participants. You’ll perform enrollment exams, diagnose illnesses, manage chronic disease, and coordinate with physicians, therapists, and social services to update care plans.

Strong communication and organizational skills are essential, along with a current APRN/PA license and CPR certification.

Qualifications

• Current APRN or PA license with Oklahoma eligibility without restriction.
• CPR certification current; able to practice per state CMS guidelines.
• Experience with frail or elderly populations preferred.

Responsabilités

• Deliver medical care to Valir PACE Foundation participants.
• Perform comprehensive exams at enrollment and semi-annually.
• Diagnose and treat acute illnesses; manage chronic diseases.
• Coordinate care with the interdisciplinary team and outside specialists.
• Document accurately and timely; participate in committees as needed.
