Valir PACE Foundation is seeking an Advanced Practice Provider (APRN/PA) to deliver medical care within an interdisciplinary team for our PACE participants. You will determine and monitor care plans in line with CMS regulations and participate in comprehensive enrollment exams and ongoing follow-ups.

The role requires current Oklahoma APRN or PA licensure, CPR certification, and experience with frail elderly populations.

Qualifications

• Current APRN or PA license.
• Certified to practice in the State of Oklahoma.
• Licensed by OBNDDC and the DEA.
• CPR certification.
• Experience with frail and elderly population is preferred.

Responsabilités

The Provider works jointly with physicians, therapy, activities, dietary, social services and transportation as part of the Interdisciplinary Team to determine and monitor the care plan for each PACE participant in compliance with all national and state regulations/laws.Duties/Responsibilities:Performs comprehensive exams on all participants upon enrollment then every six months and upon change of status in accordance with CMS regulations.Diagnose and treat acute illness in participants.Provide follow up care to manage chronic disease in participants.Engage the participants support system in medical care.Rounds/attends to institutionalized participants.Coordinates care of specialty providers, supporting good communication and providing follow-up indicated by specialty consults.Assist the IDT with the development and revision of care plansServes on committees as required by the Medical Director, Program Director and/or Board of Directors.Utilizes good mannerisms and professional demeanor with participants, families, community care providers and PACE team.Ensures positive health outcomes for participants through vigilant examinations, specialty care when needed and medication management.Carries out accurate and timely documentation.Other duties as assigned.Required Skills/Abilities:Excellent verbal and written communication skills.Excellent interpersonal and customer service skills.Excellent customer service skills.Excellent organizational skills and attention to detail.Excellent time management skills with a proven ability to meet deadlines.Strong analytical and problem-solving skills.Ability to prioritize tasks and to delegate them when appropriate.Ability to function well in a high-paced and at times stressful environment.Proficient with Microsoft Office Suite or related software.Education, Licenses, Certifications and ExperienceCurrent APRN or PA LicenseCertified to practice in the State of Oklahoma without limitation or restrictionLicensed by both the Oklahoma Bureau of Narcotics and Dangerous Drugs Control (OBNDDC) and the Drug Enforcement Agency (DEA)Either one year working with the frail and elderly population or approved training.Current CPR certificationPhysical Requirements:Ability to speak, hear, and read English.Lift up to 20lbs. from floor to 36 and from 36 to 64 (occasional)Walk up to .25 miles (occasional)Sustained forward bending in sitting (occasional)Push/Pull up to 75lbs of force at 36 (occasional)Repetitive squat motion (occasional)Ability to stand (occasional), sit (occasional), sustained squat (occasional), kneeling (occasional)Full body range of motion (constant)Front carry two handed carry of 25lbs. up to 20 feet (occasional)Grasp of various sizes and weights up to 50lbs (constant)Balance (continuous) in walking, standing, carrying, moving patients, and equipment, etc.
