Valir PACE LLC in Oklahoma City, OK is seeking a Licensed Physical Therapist for a PRN daytime role to evaluate and treat participants’ mobility and motor function. The role emphasizes goal-directed therapy to promote independence in daily activities. Travel up to 50% and collaboration with families and care teams are expected.

The candidate should have Oklahoma licensure, prior experience with frail or elderly populations, and CPR certification. Weekday availability, M-F, is preferred.

Qualifications

• Licensed Oklahoma Physical Therapist required.
• 1 year working with frail/elderly or approved training.
• Current CPR Certification required.

Responsibilities

• Evaluates and treats participants' mobility and motor assets for functional performance.
• Promotes independence in self-care, mobility and daily life tasks.
• Participates in family/participant care plan meetings and IDT.

About the RoleJob Summary:

Evaluates and treats participants mobility and motor assets and liabilities for functional performance. Provides goal directed, activity oriented treatment to promote optimum independence in self-care, functional mobility and daily life tasks. Ensures continuation of performance through effective education of the participant, family or support system. Provides adaptive methods and equipment as appropriate to enhance performance. Participates in family / Participant care plan meetings, IDT and various committees as determined by Therapy Director.

We are seeking weekday availability M-F.

Education, Licenses, Certifications and Experience:

• Licensed Oklahoma Physical Therapist
• Either at least 1 year working with the frail/elderly or approved training
• Current CPR Certification
