Job summary

Valir Health is seeking a licensed Physical Therapist for the Valir PACE program in Oklahoma City, OK. The role involves evaluating and treating senior participants to maintain and improve functional independence in daily activities.

We require licensure in Oklahoma, a background with frail/elderly populations, and CPR certification. This PRN position offers weekday availability Monday–Friday and travel up to 50% within the service area.

Qualifications

• Licensed Oklahoma Physical Therapist.
• At least 1 year working with the frail/elderly or approved training.
• Current CPR Certification.

Responsibilities

• Evaluate and treat participants’ mobility and motor assets and liabilities for functional performance.
• Provide goal-directed, activity-oriented treatment to promote independence in self-care and daily tasks.
• Participate in family/participant care plan meetings and IDT committees as determined by Therapy Director.
