Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. You will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist in good standing.
• Experience delivering patient-centered care to seniors is desirable.

Responsibilities

• Evaluate residents in skilled nursing and post-acute settings.
• Develop individualized treatment plans to improve ADLs, mobility, cognition and safety.
• Deliver evidence-based interventions and track progress in EMR.
