Job summary

Renewal Rehab seeks a licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers to support ongoing functional outcomes and quality of life.

Qualifications

• Licensed Occupational Therapist required.
• Experience evaluating residents in SNF/post-acute settings preferred.
• Ability to develop individualized treatment plans and deliver evidence-based interventions.
