Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, creating personalized treatment plans, and delivering evidence-based interventions to enhance ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active occupational therapy license in Illinois.
• Experience serving seniors in skilled nursing or post-acute facilities.
• Ability to develop individualized treatment plans and deliver evidence-based interventions.
