Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist in good standing.
• Experience with seniors in SNF or post-acute settings is preferred.

Responsibilities

• Evaluate residents in skilled nursing/post-acute facilities.
• Develop individualized treatment plans to improve ADLs, mobility, cognition, and safety.
• Deliver evidence-based interventions and educate families and caregivers.
• Document patient progress in EMR and collaborate with an interdisciplinary team.
