Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist with state licensure.
• Experience applying evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Ability to document clearly in EMR and educate families/caregivers.

Responsibilities

• Evaluate residents in skilled nursing/post-acute facilities.
