Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering interventions to improve ADLs, mobility, cognition, and safety.

You'll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active Illinois OT license required.
• Experience in geriatric care or skilled nursing preferred.
• Ability to assess residents and design targeted treatment plans.

Responsibilities

• Evaluate residents in skilled nursing and post-acute facilities.
• Develop individualized treatment plans to improve ADLs, mobility, cognition, and safety.
• Deliver evidence-based therapeutic interventions.
