Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold an active Occupational Therapist license in the state of Illinois.
• Experience in skilled nursing or post-acute settings preferred.
• Ability to document and communicate treatment plans clearly.

Responsibilities

• Evaluate residents to determine OT needs and goals.
• Develop individualized treatment plans and implement evidence-based interventions.
• Address ADLs, mobility, cognition, and safety to improve function.
• Document patient progress in EMR and adjust plans accordingly.
• Collaborate with interdisciplinary team and educate families and caregivers.
