Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This on-site role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist in Illinois.
• Experience with seniors in skilled nursing or post-acute settings preferred.
• Strong documentation and communication skills.

Responsibilities

• Evaluate residents to determine OT needs.
• Develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document patient progress in EMR and maintain accurate records.
• Collaborate with an interdisciplinary team and educate families/caregivers.
