Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist with current OT license.
• Experience in skilled nursing or post-acute settings preferred.
• Ability to develop individualized treatment plans.
• Strong documentation in EMR and interprofessional collaboration.

Responsibilities

• Evaluate residents to determine treatment needs.
• Develop individualized treatment plans for seniors.
