Job summary

Renewal Rehab in Ohio is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT with current Ohio license.
• Experience with geriatrics in SNF or post-acute settings preferred.
• Ability to perform patient-centered assessments and develop treatment plans.
