Job summary

PACE Southeast Michigan seeks a Nurse Practitioner to provide primary care for an assigned panel of participants at PACE SEMI, under the supervision of the center Physician-In-Charge. The NP handles both acute and chronic care management and coordinates with the interdisciplinary team to optimize participant outcomes.

Responsibilities include chronic disease management, regular assessments, and updating treatment plans.

Qualifications

• Must hold a current Michigan NP license and meet CME requirements.
• Geriatrics experience preferred; familiarity with frail elderly population is an asset.
• Experience documenting in SOAP format and coordinating with the care team.

Responsibilities

• Provide primary care to an assigned panel of participants under supervision of the PIC.
• Perform monthly quota of comprehensive annual and semi-annual assessments.
• Conduct Chronic Care Management visits every 3 months.
• Update management plans for changes in chronic disease or new diagnoses.
• Collaborate with physicians and team on participant status and care planning.
• Prescribe medications under physician delegation and arrange referrals as needed.
• Coordinate diagnostics, labs, imaging, and specialty procedures.
• Document notes in SOAP format with timely completion within 72 hours.
• Support risk management, directives, and goals of care discussions.
