Job summary

Volunteers of America’s Senior Community Care of Michigan PACE in Lansing, MI is seeking a full-time Nurse Practitioner to provide primary care to frail elderly participants onsite.

The NP will perform assessments, develop plans of care with the Interdisciplinary Team, and work under the Medical Director in a Program of All-Inclusive Care for the Elderly setting with regular hours and on-call rotation.

Qualifications

• RN licensed in the state of employment.
• Graduate of a professional nursing school.
• Certified as a nurse practitioner by a national certifying organization.
• Minimum of one year experience with frail/elderly population.

Responsibilities

• Provide primary care to enrolled participants.
• Perform physical assessments for new enrollees and semi-annual reassessments.
• Develop and implement plans of care with the Interdisciplinary Team.
• Educate participants and caregivers on health management.
• Coordinate care with Primary Care Physicians and Medical Directors.
