PACE SEMI in Dearborn, MI seeks a part-time Nurse Practitioner to provide primary care, chronic disease management, and preventative services under physician supervision. Responsibilities include comprehensive assessments, coordination with the interdisciplinary team, and facilitating goals of care with participants and families.

The NP will document in SOAP format, assist in utilization management, and participate in after-hours calls as needed.

Qualifications

• Nurse Practitioner with licensure in Michigan.
• Experience with geriatrics preferred; 3–5 years in clinical setting.
• Must fulfill CME requirements and TB screenings, infection control, BLS.

Responsabilités

• Provide chronic disease management and primary care to an assigned panel.
• Perform annual and semi-annual assessments.
• Coordinate care with IDT, PCT, UM, and PACE services; document in SOAP format.
• Facilitate advanced directives and end-of-life discussions with participants and families.
