Job summary

PACE SEMI in Michigan is seeking a Nurse Practitioner to provide primary care for an assigned participant panel. The NP will manage acute and chronic care under supervision of the center PIC, perform regular assessments, and coordinate care with the interdisciplinary team.

Responsibilities include chronic disease management, pharmacologic prescribing delegated by the physician, preventive care, and documentation in SOAP format within 24–72 hours.

Qualifications

• 3–5 years of clinical nursing experience in geriatrics preferred.
• Must hold Michigan NP license in good standing.
• Fulfills CME requirements and TB screenings.

Responsibilities

• Provide primary care to an assigned participant panel.
• Perform monthly comprehensive annual and semi-annual assessments.
• Manage chronic disease and update care plans.
• Prescribe medications as delegated by supervising physician.
• Coordinate with PIC and interdisciplinary team (IDT).
• Document in SOAP format within 24–72 hours.
