Job summary

PACE Southeast Michigan is seeking a Nurse Practitioner to deliver primary care to an assigned panel of participants at PACE SEMI. The NP will manage chronic and acute care under supervision of the center PIC and coordinate with the interdisciplinary team.

The role emphasizes geriatrics experience, CME compliance, and maintaining a Michigan NP license, with potential after-hours coverage as needed to support participants.

Qualifications

• Three (3) to five (5) years of progressively more responsible experience as an RN in a clinical setting with geriatrics expertise preferred.
• Must have one (1) year of experience with a frail or elderly population.
• Maintain current Michigan NP License in good standing and fulfills CME requirements.

Responsibilities

• Provide primary care for an assigned participant panel.
• Perform monthly quota of comprehensive annual and semi-annual assessments.
• Conduct Chronic Care Management visits every 3 months.
• Update management plans for changes in chronic disease or new diagnoses.
• Collaborate with Physician and clinical team on participant status.
