Job summary

PACE SEMI in Westland, MI is seeking a Nurse Practitioner to provide primary care and manage chronic conditions for an assigned participant panel. You will work under the supervision of a center physician and may practice independently within your license scope.

Responsibilities include comprehensive assessments, medication management, care coordination with interdisciplinary teams, and facilitating goals of care and end-of-life discussions when appropriate.

Qualifications

• Three to five years of progressively responsible experience as an RN in a clinical setting with geriatrics focus.
• One year of experience with a frail or elderly population.
• Maintain current Michigan NP License and CME requirements.
• The NP may practice independently within license scope under supervision as needed.

Responsibilities

• Provide primary care to an assigned participant panel with chronic and acute care management.
• Perform monthly comprehensive and semi-annual assessments; document notes in SOAP format.
• Prescribe medications delegated by supervising physician; manage chronic conditions and update care plans.
• Coordinate care with the IDT, PCT, UM, and other PACE services; communicate changes.
• Facilitate goals of care, advance directives, and end-of-life discussions with participants and families.
• Manage after-hours on-call medical assistance on a rotating basis; participate in education conferences.
