Job summary

Nurse Practitioners Services of Texas provides care to nursing home residents enrolled in the Texas Independence Health Plan. The NP delivers primary and acute care management for approximately 60–70 patients in two nursing homes, in collaboration with an RN Care Coordinator.

The NP establishes individualized care goals and facilitates communication among members, families, providers and facility staff. They set advance care preferences and educate on disease progression to prevent

Qualifications

• MSN required.
• Texas NP licensure required.
• Current NP certification required.
• DEA prescriptive license required.
• Ability to work independently and as part of a team.

Responsibilities

• Provide primary and acute care management for 60–70 patients in two nursing homes in collaboration with an RN Care Coordinator.
• Establish individualized care goals for each member.
• Facilitate communication between members, families, providers and facility staff.
• Establish advance care preferences with each member.
• Educate patients about disease progression and early signs of change.
• Manage members in skilled nursing facilities to avoid unnecessary hospitalizations.
