Job summary

Nurse Practitioners Services of Texas provides care to nursing home residents enrolled in the Texas Independence Health Plan. The NP delivers primary and acute care management for approximately 60–70 patients in two nursing homes, in collaboration with an RN Care Coordinator.

The NP establishes individualized care goals and facilitates communication among members, families, providers and facility staff. They set advance care preferences and educate on disease progression to prevent

Job description

Clinicians of Nurse Practitioners Services of Texas provide care to nursing home residents enrolled in an Institutional Special Needs Plan called Texas Independence Health Plan. Provides primary care and acute care management for approximately 60-70 patients in 2 nursing homes in collaboration with an RN Care Coordinator. The emphasis is on proactive and preemptive care through general health management, increased availability of on-site clinicians, and early identification of changes in condition to avoid unnecessary hospitalizations and ED visits. The NP establishes individualized care goals for each member and facilitates communication between members, families, providers, and facility staff, and establishes advance care preferences with each member. We strive to provide education about disease progression, address problems sooner in the disease process, and manage members in the skilled nursing facility whenever possible.

This position will support 4 locations: Alice, San Diego, Kingsville, and Robstown, Texas.
