Job summary

CareMed Health Solutions is seeking Nurse Practitioners to deliver primary and preventative care to residents in skilled nursing facilities. You will manage acute changes, participate in advanced and chronic care planning, and collaborate with an interdisciplinary care team.

Applicants must hold an NH NP license (unrestricted), board certification, and a DEA, with experience in geriatric or adult care and EMR systems.

Qualifications

Current, unrestricted NP license in NH
Board Certification
DEA licensure
Clinical background in adult, family or geriatric specialties
Experience in long-term care setting
Ability to work with an EMR
Ability to manage patients with advanced illness and multiple chronic conditions
Ability to work within an interdisciplinary care team
Psych experience a plus

Responsibilities

Provide primary and preventative care for long term care patients in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe medications, including controlled substances per NH law.
Order, review and follow up on treatments and diagnostic testing.
Communicate with nursing home staff regularly about changes in condition and care plan.
Collaborate with primary care physician.
