Job summary

CareMed Health Solutions is seeking a licensed Nurse Practitioner to deliver primary and preventive care to residents in skilled nursing facilities in Kentucky. The NP will manage acute changes, perform advanced care planning, and coordinate with the facility and primary care physicians to optimize patient outcomes.

Candidates must hold current KY NP licensure, board certification, and DEA; experience in long-term care and EMR proficiency are required.

Qualifications

Current, unrestricted NP license in Kentucky.
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience in long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and chronic conditions.
Ability to collaborate within an interdisciplinary care team.

Responsibilities

Provide primary and preventive care for long-term care patients in skilled nursing facilities.
Manage residents with acute changes in condition.
Perform advanced care planning and chronic care management.
Prescribe medications, including controlled substances per KY law.
Order, review and follow up on treatments and diagnostics.
Communicate regularly with nursing home staff about patient changes.
Collaborate with the primary care physician.
