Job summary
CareMed Health Solutions seeks a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities in Kentucky. You will manage acute changes, plan advanced care, and coordinate with the interdisciplinary team to optimize patient outcomes.
Ideal candidates hold an active KY NP license, board certification, and DEA licensure, with experience in geriatric care and EMR systems.
Qualifications
- Current, unrestricted NP license in the state of KY.
- Board Certification in family, adult, or geriatric NP.
- DEA licensure to prescribe controlled substances.
- Experience in a long-term care setting.
- Ability to work within an interdisciplinary care team.
- Experience with an EMR system.
Responsibilities
- Provide primary and preventative care for long-term care patients.
- Manage residents with acute changes in condition.
- Advanced care planning and chronic care management.
- Prescribe medications, including controlled substances per KY law.
- Order, review, and follow up on treatments and diagnostics.
- Communicate with nursing home staff about changes in condition or care plan.
- Collaborate with the primary care physician.
