Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. The NP will manage acute changes, participate in advanced care planning and chronic care management, and collaborate with primary care physicians and nursing home staff.

Applicants must hold an unrestricted Kentucky NP license, board certification, DEA licensure, and have experience in adult, family, or geriatric care, preferably in a long-term care setting,

Qualifications

• Active NP license in Kentucky.
• Board certification as an NP.
• DEA licensure is required.
• Experience in adult, family or geriatric care.
• Experience in long-term care settings is preferred.
• Familiarity with EMR systems.
• Ability to manage complex chronic conditions.

Responsibilities

• Provide primary and preventive care for long-term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Engage in advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow-up on treatments and diagnostic testing.
• Communicate with nursing home staff about changes in condition and care plans.
• Collaborate with the primary care physician.

Education

Current, unrestricted NP license in Kentucky

Board Certification

DEA licensure

Clinical background in adult, family or geriatric specialties

Experience in long-term care setting

EMR proficiency

Ability to manage advanced illness and multiple chronic conditions

Ability to work in an interdisciplinary care team
