Job summary

CareMed Health Solutions seeks a Nurse Practitioner to provide primary and preventative care for residents in skilled nursing facilities. You will manage acute changes in condition and engage in advanced care planning and chronic disease management.

Responsibilities include prescribing medications per OH law, coordinating treatments, and collaborating with primary care physicians within an interdisciplinary team to ensure high-quality geriatric care.

Qualifications

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advance care planning and chronic disease management.
• Prescribe medications, including controlled substances per OH law.
• Order, review and follow up on treatments and diagnostic testing.
• Communicate with nursing home staff about changes in condition and care plans.
• Collaborate with primary care physicians.
