Job summary

PACE Southeast Michigan seeks an experienced Nurse Practitioner to deliver primary care and chronic disease management to a panel of participants. Under supervision of the center Physician-In-Charge, you will perform comprehensive assessments, coordinate care, and facilitate preventative and end-of-life discussions.

Responsibilities include conducting quarterly chronic care visits, ordering diagnostics with physician oversight, and ensuring timely documentation in SOAP format.

Qualifications

• Three to five years of RN experience with geriatrics preferred.
• Must hold active NP license in Michigan and CME requirements.
• Ability to collaborate with interdisciplinary teams and coordinate care.

Responsibilities

• Provide primary care for an assigned participant panel.
• Manage chronic and acute conditions with physician supervision.
• Perform annual and semiannual assessments monthly quotas.
• Document in SOAP format within 24–72 hours.
