Job summary

Senior Community Care of Michigan PACE, part of VOANS, is seeking a Full-Time Nurse Practitioner to deliver primary care to frail elderly participants at our Lansing-based program.

Under Medical Director supervision, you will perform physical assessments, develop care plans with the Interdisciplinary Team, and provide education to participants and caregivers. Schedule is Monday–Friday, 8:00a–4:30p with rotating after-hours on-call; occasional weekends; full onsite.

Qualifications

• Licensed RN in state of employment.
• Graduate of a nursing program.
• Certified as a nurse practitioner by a national certifying organization.
• Minimum of one year experience with frail elderly population.

Responsibilities

• Provide primary care to Senior Community Care enrollees.
• Perform physical assessments of new enrollees and semi-annual reassessments.
• Develop and implement plans of care with the Interdisciplinary Team.
• Educate participants and caregivers on treatment plans.
• Coordinate care with Primary Care Physicians and Medical Directors.
• Conduct in-person histories and physicals; assist with discharge planning as needed.
