Qualifications

• APRN/PA license required
• 3 years' experience in geriatric clinical nursing or rehab
• Masters degree or better in a related field
• Experience with Electronic Health Records and MS Office

Responsibilities

• Assess patients' medical and mental health needs and develop care plans
• Prescribe medications and order labs and consultations
• Coordinate with RN case managers and facility staff
• Ensure compliance with federal and state regulations
• Provide team-based, patient-centered care in nursing homes
