Job summary
Providence PACE is seeking a full-time Nurse Practitioner or Physician Assistant to join our Portland, Oregon team, focusing on primary care for older adults in a team-based setting. Many visits occur in homes and community-based settings in addition to outpatient clinics.

Qualifications
- 1 year of post-graduate clinical experience in geriatrics preferred.
- Training/experience in palliative care and/or hospice is valuable.
- Ability to collaborate effectively with an interdisciplinary care team.
Responsibilities
- Provide primary care to older adults across outpatient clinics and home-based settings.
- Collaborate with physicians, nurse practitioners, care managers, social workers, pharmacists, and therapists.
- Deliver comprehensive care including medical, behavioral health, and supportive services.
