Job summary
Providence PACE in Portland, Oregon is seeking a full-time nurse practitioner or physician assistant to join a nationally recognized program delivering comprehensive, team-based care for older adults in diverse settings, including outpatient clinics and community residences. Qualifications
- 1 year of post-graduate clinical experience working with a geriatric population preferred
- Training and/or experience in palliative care and/or hospice valuable
- Strong ability to collaborate with an interdisciplinary team
Responsibilities
- Care provided in a variety of settings, primarily community-based but also outpatient clinics
- Collaborate closely with physicians, nurse practitioners, nurse care managers, social workers, pharmacists, psychiatric/mental health clinicians, therapists, and other team members
- Join an established, interdisciplinary program focused on comprehensive whole-person care for older adults
