Job summary

Providence PACE in Seaside, Oregon is seeking a full-time Nurse Practitioner or Physician Assistant to join its interdisciplinary team providing comprehensive primary care for older adults across clinic and home-based settings.

Ideal candidates have interest or experience in geriatrics and palliative care, with a preference for at least one year of geriatric clinical experience. Relocation assistance, CME funds, and a strong benefits package are offered.

Qualifications

• One year of post-graduate clinical experience working with a geriatric population preferred.
• Training and/or experience in palliative care and/or hospice valuable.

Responsibilities

• Provide care in outpatient clinics and various community-based settings including private residences, assisted living facilities, adult care homes, and skilled nursing facilities.
• Collaborate closely with physicians, nurse practitioners, nurse care managers, social workers, pharmacists, psychiatric/mental health clinicians, therapists, and other interdisciplinary team members.
• Build trust quickly with patients, families, and care teams.
