Job summary

Visionary Healthcare Partners in Roseburg, OR is seeking a Nurse Practitioner or Physician Assistant to care for medically complex older adults in outpatient clinics and home visits. You will join an interdisciplinary team, manage chronic conditions, support hospital transitions, and ensure accurate medical records.

This role requires Oregon NP licensure with ANCC/AANP certification, 2+ years in primary care, and ability to travel throughout the Salem area.

Qualifications

• Oregon NP license and ANCC/AANP certification required.
• Experience with high-risk patients, acute care, home health, or underserved populations preferred.
• At least two years of NP primary care experience (family/internal/gerontology).
• Knowledge of chronic-care management, Medicare HCC coding, and managed care.
• Active CPR/BLS certification and a current Oregon driver's license.
• Ability to obtain DEA registration and prescriptive authority after hire.
• Ability to travel throughout the Salem area five days per week.

Responsibilities

• Care for medically complex older adults in outpatient clinics and homes.
• Conduct Medicare wellness visits, review histories, perform screenings, and educate patients.
• Complete HCC coding and risk-adjustment documentation; maintain medical records.
• Schedule based on 10–12 visits/day in clinic and 5–6 home visits; travel within ~30 miles.
• Collaborate within interdisciplinary care teams with care coordinators and physician advisors.
