Job summary

AlignMed Partners seeks a full-time Nurse Practitioner for our Jersey Shore Skilled Nursing and Rehabilitation Center in Jersey Shore, PA. This 40-hour-per-week role offers a base salary of $110,000-$130,000/year plus a no-cap incentive compensation program.

The Advanced Practice Provider delivers primary and urgent care in collaboration with Center Leadership and the Medical Director, with strong clinical leadership support and access to UpToDate, CAPC, and Nabla AI tools.

Qualifications

• NP with Master's Degree in Nursing and licensed as RN
• Nurse Practitioner certification with Older Adult and/or Family NP focus

Responsibilities

• Provide primary and urgent care to nursing center residents in collaboration with other providers
• Collaborate with Center Leadership to meet goals and priorities
• Coordinate care with Medical Director/Attending Physician and maintain documentation
