Job summary

San Ysidro Health is seeking a Registered Nurse to contribute to the SD PACE program as part of an interdisciplinary team. You will provide direct and indirect nursing care, manage complex clinical situations, and collaborate with multiple disciplines to ensure patient safety and quality care.

The role requires independent application of advanced nursing knowledge, travel to various sites when needed, and coordination with the IDT to support comprehensive care for frail older adults.

Qualifications

• 6 months experience working with the frail and/or elderly population.
• Knowledge of care paths and interactive tools in skilled nursing/elder care settings.
• Ability to triage clinical symptoms and coordinate with physicians.
• Comfort with starting IV lines and IV medication administration.
• Experience with acute care of frail adults and cognitively impaired patients.

Responsibilities

• Assess patient data and set priorities using the nursing process.
• Develop and revise plans of care with IDT involvement and family input.
• Provide and document advanced nursing care and patient education.
• Record observations and maintain thorough nursing documentation.
• Lead nursing activities, delegate tasks, and support staffing needs.
• Participate in IDT meetings and contribute to care planning.
