Job summary

North East Medical Services is seeking a PACE Clinic RN in San Francisco to provide direct and indirect nursing care under the supervision of the PACE Center Manager. You will work collaboratively with physicians and the IDT to manage ongoing care for Participants at the PACE Center.

Responsibilities include conducting assessments, documenting care, and supporting enrollment and education. The role may involve after-hours on-call coverage and telehealth as needed.

Qualifications

• Valid California RN license.
• Graduate of an accredited school of Nursing.
• Current BLS and First Aid required.

Responsibilities

• Conduct nursing assessments to determine care needs and goals.
• Participate in IDT care planning for Participants.
• Deliver and document nursing interventions per care plans.
• Coordinate with care team for smooth transitions between settings.
• Provide health education to Participants and families.
• Support enrollment in the PACE program and end-of-life care coordination.
• Document all care in the electronic health record accurately.
