Job summary

PACE Southeast Michigan seeks a Nurse Practitioner to provide primary care and chronic disease management for an assigned participant panel. Under supervision of the PIC, you will perform annual/semi-annual assessments, manage chronic conditions, and coordinate with the interdisciplinary team to optimize care.

Qualifications

• Three to five years of experience as an RN in a clinical geriatrics setting is preferred.

Responsibilities

• Provide primary care for an assigned panel of participants.
• Perform chronic disease management and care planning.
• Conduct comprehensive annual and semi-annual assessments.
• Prescribe medications under physician delegation and coordinate referrals.
• Educate patients and families; document in SOAP format.
